Sex Differences in Cognitive Performance in Alzheimer's Disease: Insights from the ADAS-Cog-13

Sex differences in Alzheimers disease (AD) are well recognized, yet their impact on cognitive assessment remains underexplored. We examined sex differences in the 13-item AD Assessment Scale-Cognitive Subscale (ADAS-Cog-13) separately within cognitively normal (CN), and AD patients, controlling for hippocampal atrophy. CN women performed significantly better than men on the total score, word recall, naming, and word recognition. No sex differences were observed within the AD group on the total score, but males performed better in delayed word recall. These findings underscore the need to account for sex when interpreting cognitive test results, especially in preclinical stages of AD.
