Mindfulness-Based Interventions using Artificial Intelligence: A Systematic Review Protocol

BackgroundMindfulness-based interventions (MBIs) have gained significant recognition as effective approaches for promoting mental health and well-being. With rapid advancements in artificial intelligence (AI), there is growing interest in leveraging AI technologies to enhance the delivery, personalization, and effectiveness of MBIs. However, the current state of research in this field remains nascent and fragmented, with limited comprehensive understanding of the types of AI technologies being used, their effectiveness in promoting mental health outcomes, and user experiences with these interventions. ObjectivesThis systematic review aims to comprehensively synthesize existing literature on AI-based MBIs. The primary objectives are to: (1) identify and characterize the types of AI technologies employed in MBIs, including machine learning, natural language processing, computer vision, and other relevant approaches; (2) evaluate the effectiveness of these interventions in promoting mental health and well-being outcomes, such as reducing symptoms of depression, anxiety, and stress, and enhancing overall quality of life; (3) examine the acceptability, feasibility, and user experience of AI-based MBIs; and (4) identify key trends, research gaps, and challenges within the current literature to provide recommendations for future research and practice. Methods and AnalysisThis protocol follows PRISMA-P guidelines and has been registered in PROSPERO (CRD42025641273). Eligibility criteria are based on the PICOS framework, including adults ([≥]18 years) with or without mental health conditions, mindfulness-based interventions incorporating AI technologies, various comparison groups (including no comparison), mental health and well-being outcomes, and diverse study designs including RCTs, non-randomized studies, and qualitative studies. A comprehensive search strategy will be implemented across multiple databases (PubMed, Web of Science, Embase, CINAHL, MEDLINE, The Cochrane Library, PsycINFO, and Scopus) from inception to January 2025, supplemented by manual searches, gray literature searches, and expert consultation. Two independent reviewers will conduct study selection, data extraction, and risk of bias assessment using appropriate tools (RoB 2 for RCTs, ROBINS-I for non-randomized studies, CASP for qualitative studies). A narrative synthesis will be conducted following Cochrane guidance, with meta-analysis considered if studies are sufficiently homogeneous. The GRADE approach will be used to assess certainty of evidence. DiscussionThis systematic review will provide the first comprehensive synthesis of evidence on AI-based MBIs, addressing critical gaps in understanding the integration of AI technologies in mindfulness interventions. The findings will inform the development of best practices and guidelines for designing, evaluating, and implementing AI-enhanced MBIs. By identifying the most promising AI technologies and applications, as well as key research gaps and methodological challenges, this review will guide future research priorities and funding allocation. The results will contribute to ongoing dialogue between researchers, practitioners, and policymakers at the intersection of mindfulness, mental health, and AI, ultimately supporting the broader goal of leveraging AI technologies to enhance the accessibility, effectiveness, and personalization of mindfulness-based interventions for mental health and well-being.
